Hypertenzí trpí už i děti a mladí lidé

Vysoký krevní tlak už není jen strašákem dospělých lidí a seniorů. Trpí jím i děti a dospívající do 18 let. Loni se s hypertenzí léčilo 3600 klientů Všeobecné zdravotní pojišťovny v této věkové skupině, jedná se o 3 děti z 1000. Mezi hlavní příčiny vzniku hypertenza patří podle odborníků nadváha, obezita a málo pohybu.

Závažné následky, jako jsou infarkt, mrtvice nebo poškození ledvin, se obvykle projeví až po mnoha letech, a pacient tak o riziku pro své zdraví často dlouho vůbec neví.

»Hypertenze u dětí může probíhat zcela bez příznaků, kdy ji náhodně odhalí většinou preventivní prohlídka u pediatra. Až u těžších forem nemoci pozorujeme varovné symptomy, kdy si děti stěžují např. na bolesti hlavy, krvácení z nosu, únavu apod.,« uvedl v dnes zveřejněné tiskové zprávě náměstek ředitele VZP ČR pro zdravotní péči Jan Bodnár. V průběhu preventivní prohlídky lékař mimo základních vyšetření zjišťuje i rodinnou anamnézu malého pacienta. »Nejde jen o výskyt samotné hypertenze u rodičů či prarodičů, ale i dalších souvisejících onemocnění, která s vysokým tlakem mohou souviset, typicky například cukrovka či onemocnění ledvin,« dodal.

Podle odborníků mezi hlavní příčiny vzniku hypertenze u dětí patří především nadváha a obezita, což představuje jeden z nejvýznamnějších rizikových faktorů. Aktuálně téměř 30 % dětské populace v České republice trpí nadváhou.

Děti mají málo pohybu

Dalším z faktorů je nízká fyzická aktivita, kdy pravidelný pohyb a sportovní aktivity bývají nahrazovány pasivním trávením času u počítače, mobilních telefonů či televize. »Tělesná hmotnost pozitivně koreluje s hodnotami krevního tlaku a představuje jednu z hlavních příčin hypertenze u dětí. Zvláště výraznou souvislost s krevním tlakem vykazuje abdominální (břišní) obezita, měřená obvodem pasu. Děti a dospívající s nadváhou nebo obezitou mají prokazatelně vyšší hodnoty krevního tlaku než jejich vrstevníci s normálním BMI, a tedy i vyšší výskyt hypertenze,« konstatoval lékař Tomáš Seeman, který se dětskou hypertenzí dlouhodobě zabývá. 

Ke vzniku hypertenze přispívá také nevhodná strava s nadměrným množstvím soli, tuků a cukrů, často pocházejícím z průmyslově zpracovaných potravin. V neposlední řadě hraje roli i stres a psychická zátěž – děti a dospívající čelí rostoucímu tlaku na výkon, sociálnímu porovnávání a nadměrnému působení digitálního prostředí, což vytváří nové formy chronického stresu už v útlém věku. Podle dostupných dat trpí hypertenzí přibližně tři z každého tisíce klientů VZP ČR mladších 18 let.

Projevy hypertenze

Vysoký krevní tlak u dětí může probíhat zcela bez příznaků, především u starších dětí a dospívajících, pokud nejde o těžkou formu. V takových případech bývá nejčastěji zachycen náhodně při preventivní prohlídce, kde by měl být krevní tlak pravidelně měřen už od tří let věku. U dětí s výraznější formou jsou projevy častější – typicky se objevují bolesti hlavy, krvácení z nosu, únava, zvýšené pocení, případně křeče nebo poruchy vědomí.

Zcela odlišná je situace u kojenců a novorozenců, kde se vysoký tlak téměř vždy projevuje a může mít i závažný nebo život ohrožující průběh. Mezi varovné příznaky patří neklid, potíže s krmením, nepřibývání na váze, dýchací potíže, křeče či známky selhávání srdce.
